I like the simplicity of the inline buttons idea. The trick is in the implementation. Perhaps they could be shown or hidden at the user's discretion. If you check the "Do not ask again for this document as long as it remains open" box, then to get the dialog back, you would need to close that document and reopen it. (Naturally you couldn't know this if you didn't read the dialog carefully before you checked the box.)

Another way is to set the document up with a different first-page header. (Check "Different First Page" under Header and Footer in the Sections palette.) Then add returns to the end of the first-page header enlarge it. The advantage here is that you don't have to put a forced break in your document....
