I have a 500+ item list of dictionary items
How do I write a macro or regex to select all the first words in the lines?

You can do this easily with Powerfind (or Powerfind Pro).
In the Find box do:
- Switch to Powerfind
- From Special Positions choose Start of Paragraph
- From Wild Card choose AnyWord
- Do Find All
