Indexing: how do I actually insert the index into doc??

I've been using NWP happily for a few months, but I'm stuck on indexing. I can set up the index words, but I can't figure out how to actually create an index inside the document!
I'm following the manual page 245 where it says:
Insert an index into your document
1. Put your insertion point (at the end of your document) where you want the index to appear.
2. Choose the menu command Tools > Index > Insert Index....
But whatever I do, the Insert Index is grayed out. Apparently I have to do something special with the cursor? I have tried putting it at the end, after all the footnotes, but to no avail. Is there some special sequence I need to follow? (I am using "Index using word list," and it reports 280 hits.)
I must be overlooking something "obvious." Any suggestions? :?

Update: I discovered the problem was where I was trying to put the index. Apparently, the index must be before the endnotes. This violates standard practice, of course, but I can live with it.

However, now I am running into the same weird "changing page numbers" problem.

So I have temporarily given up on indexing entirely. :(
Fortunately it's not critical for me, but with a 100 page working paper, I figured....

You can insert the index to appear after the endnotes.

To do that, in the Endnote style, in Style Sheet, set the endnotes to appear at the 'End of Section' (rather than 'End of Document'):

Then, before inserting the index, insert a section break.
