Tab insert labels - table and paper size setup

I want to print a tab insert strip that is 3.25" wide and 9.25" long with 10 rows, each row allotting .5" for text and .25" above that for padding.
How do I format this both as far as the table labels, and also to print it by feeding it through the center of a paper tray?
I don’t know whether to focus on section top and bottom margins, table position, page setup, or otherwise. Assuming inner and outer margins that are each one inch, I’ve tried variations of a centered table with left and hanging indent of 2.67" and right indent of 5.93" (which yields 3.26", .01" over 3.25" which is okay).
