Numbering ToC (chapter) titles (best practice?)

I'm working on a book with about 12 chapters. The chapter titles are assigned to the ToC. However, since those titles do not include the chapter numbers, when I create the ToC, it's just a list of chaper titles in order. What's the best practice to get the chapter numbers to show up? Note also that the Foreward does not, of course, have a chapter number. I'd like the ToC to look something like this:
