Problems with Multicolumn Sections

Hi.
For some reason when I set up 2 columns per page the left hand column of the first page is blank. Subsequent pages have text in both columns. This problem does not appear to occur with automatically generated multi-column indices.
I have another sporadic problem. Sometimes blank pages are introduced into the middle of my file without me introducing a section break or page break of any kind. Sometimes cutting and pasting text or retyping text fixes the problem.
