Centering within a table

I have three column single row table that I use for some equation numbering. The small width column on the left is an equation type label. The small width column on the right is a list counter. The center column of width about 5 inches is for the equation graphic.

OK so far, now I center the equation graphic from MathEq (the old expressionist) and it looks great. The problem is that when I save the document some, not all of these entries shift to right-hand centering. I fix them and it occurs again.

So do I fix this!

I tried creating a table that fits your description and didn't have any trouble with lost alignment. However, I may have misunderstood, or my document may not be complex enough. Can you please send us your document using the menu Help > Send Feedback? Thanks.
